卵巢上皮性癌腹水与预后的关系

THE PROGNOSTIC SIGNIFICANCE OF ASCITES IN PATIENTS WITH EPITHELIAL OVARIAN CANCER

目的 探讨卵巢上皮性癌(EOC)腹水与预后的关系。 方法 回顾性分析1986年～1993年在我院施行卵巢癌肿瘤细胞减灭术的101例初治卵巢上皮性癌患者的临床病理及随访资料。根据术中所见有无腹水将患者分为两组,腹水组62例和无腹水组39例。比较两组患者的平均年龄、病理类型、晚期患者所占的比例以及患者的生存率在统计学上有无差异。 结果 两组患者的平均年龄、病理类型在统计学上均无显著差异。但是,无腹水组晚期患者所占的比例明显少于腹水组;无腹水组患者的三、四、五年生存率显著高于腹水组患者;无腹水组Ⅲ期患者的五年生存率显著高于腹水组Ⅲ期患者。

PURPOSE To investigate the prognostic significance of ascites in patients with Epithelial Ovarian Cancer (EOC).METHODS A retrospective review of 101 patients who first underwent tumor debulking surgery for epithelial ovarian cancer in our hospital between January 1986 to December 1993 was performed.The patients were divided into two study groups based upon the presence or absence of ascties atfirst laparotomy.Ascites group:62 patients;No ascites group:39 patients.The mean age of patients,cell type,proportion of advanced stage patients or survival were compared between the groups by a χ 2 test.RESULTS No statistical difference in mean age of patients,cell type was noted.However,more patients with advanced stage disease had ascites than those without ascites( P <0 01). The 3 ,4 and 5 year survival in the no ascites group were 87.02%,73.42%,57.10% respectively versus 65.02%,38.66%,28.12% respectively in the ascites group.The 5 year survival of stage Ⅰ,Ⅱ,Ⅲ,Ⅳ patients in no ascites group were 77%,70%,41.18%,0 respectively versus 60%,56.8%,15.46%,0 respectively in the ascites groups.The result showed that 3 ,4 and 5 year survival in no ascites group were significantly higher than those in ascites group( P <0 01).The 5 year survival of stage Ⅰ,Ⅱ in no ascites were higher than those in ascites group( P >0 05),while the 5 year survival of stage Ⅲ in no ascites was significantly higher than those in ascites( P <0 01) CONCLUSIONS The ascites is a significant poor prognostic factor of EOC.
