摘要
目的 研究癌胚抗原(CEA)、细胞角质蛋白19(CYFRA211)和腺苷脱氨酶(ADA)对恶性胸腔积液的诊断意义。 方法 测定58例恶性胸腔积液及26例结核性胸腔积液的CEA、CYFRA211、ADA的水平。 结果 恶性组胸水CEA、CYFRA211水平均明显高于结核组(P<0001)。恶性组胸水ADA值明显低于结核组(P<0001)。胸水CEA对诊断恶性胸腔积液的敏感性为8103%,特异性为9615%。CYFRA211的敏感性为9655%,特异性为7308%。ADA的敏感性为8750%,特异性为9642%。 结论 联合检测胸水CEA、CYFRA211、ADA的水平。
PURPOSE The significance of carcinoembryonic antigen(CEA),CYFRA21 1 and adenosine deaminase (ADA) in the diagnosis for malignant pleural effusions was studied.METHODS CEA,CYFRA21 1 and ADA levels in pleural fluid from 58 patients with malignant pleural effusions and 26 patients with tuberculous pleural effusions were measured.RESULTS In the malignant group,the fluid CEA and CYFRA21 1 levels were significantly higher than that in tuberculous group( P <0 001).The malignant group had significantly lower ADA levels in pleural fluid than those of tuberculous group ( P <0 0001)。The sensitivity and specificity of pleural fluid CEA level in diagnosis malignant effusions were 81.03% and 96.15% respectively. The sensitivity and specificity of CYFRA21 1 were 96.55% and 73.08% respectively.And the sensitivity and specificity of ADA were 87.50% and 96.42% respectively.CONCLUSIONS Combined analysis of fluid CEA,CYFRA21 1 and ADA may be of help in the diagnosis of malignant pleural effusions.
