新医改形势下护理人员心理健康影响因素研究

Studying on the factors to influence the mental health of nursing staffs under the New Medical Reform

目的:通过对某综合类医院护理人员心理健康现状的调查,了解其心理健康水平及影响因素,进而提高其心理健康水平。方法:利用心理健康自评量表SCL-90与压力源量表,采用分层随机整群抽样的方法,对某综合类医院的部分护理人员进行调查,运用SPSS19.0与SAS9.1统计软件进行数据分析,统计分析方法主要采用了t检验、方差分析、Logistic回归分析及决策树分析法。结果:该院护理人员心理健康各因子得分与全国常模比差异均具有统计学意义(P<0.05);不同年龄之间抑郁、躯体化、焦虑、敌对、人际关系、偏执和恐惧7个因子的得分均具有统计学差异(P<0.05);编制和职称结构对心理健康的部分因子有影响,不同文化程度间各因子差异均无统计学意义。而从多因素分析看,Logistic回归方法优于决策树模型,确定学历、工作量及时间、躯体化、强迫症、人际关系、焦虑、敌对、恐惧、精神病性、其他是影响心理健康的主要影响因素。结论:该人群心理健康水平低于全国常模,提示我们在进行医疗卫生改革的同时多关注医护人员的心理健康问题,更多更全面的去了解其心理需求,进而改善其心理健康水平,提高医疗服务水平和医疗质量。

Objective To study the status of mental health of nursing staffs and its influencing factors. Methods The nursing staffs in a comprehensive hospital were chosen with stratified random cluster sampling and surveyed with SCL-90 and pressure source scale. Data was analyzed with statistical software of SPSS19. 0 and SAS9. 1. Results The score of each factor of mental health of the nursing staffs was significantly higher than the national norm( P < 0. 05). The seven factors of depression,somatization,anxiety,hostility,interpersonal relationship,paranoia and fear were significantly different among different ages( P < 0. 05). The identity and professional title had part influence on the score of some factors of mental health. But the differences of each factor among different education background had no statistical significance( P > 0. 05). Multiple factors analysis showed that Logistic regression method was better than decision tree model. Factors of education background,workload and time,somatization,obsessive-compulsive,interpersonal relationship,anxiety,hostility,fear and psychotic were the main factors to influence mental health. Conclusion The status of mental health of nursing staffs was worse than the national norm,which needs to be further cared to improve.
