Surgical Management of Complicated Cranio-orbital Anormalies

摘要 目的 探讨治疗各类颅眶部畸形所致的眼眶容积异常、额眶错位、眼水平轴失衡、眼球凹陷、复视的方法。方法取头皮冠状切口入路,帽状腱膜层分离头皮瓣,在眶上缘切开骨膜后显露额颅和眼眶的四壁;按不同设计,行单个颅眶分层截骨、固定,或多个颅眶复合分层截骨、固定。结果44例患者中眶上缘截骨5例,眶内壁截骨8例,眶下缘截骨3例,眶外缘截骨28例,其中眶外下缘复合截骨26例。手术前后测量结果显示,骨性眶间距(IOD)、突眼度、双眶连线与双耳屏连线的交角(A-OT)、以及外眦与耳屏连线(LC-T)等均明显改善。结论头皮冠状切口入路的颅眶分层截骨术治疗颅眶部骨架异常的整复效果优于单纯充填术。 Objective To discuss the surgical management of abnormal orbital volume,fronto-orbital malposition, imbalance of the horizontal eye axis,enophthalmos and diplopia resulting from cranio-orbital anormalies.Methods Through a transcoronal incision,the scalp flap was elevated above the periosteum,and the periosteum was incised at the upper orbital rim and elevated for the exposure of frontal bone and four- wall of the orbit.Then,simple or compound fronto-orbital layer osteotomy was performed,following by fixation.Results 44 cases in this series included 5 cases of supra-orbital osteotomies,8 medial orbital osteotomies,3 infra-orbital osteotomies,2 lateral orbital osteotomies,26 inferior-lateral orbital osteotomies.Pre- and postoperative measurements showed a significant improvement in the esthetic appearance.Conclusion This procedure can achieve a better effect than the simple implantation operation for the treatment of cranio-orbital skeleton anormalies.
