Coagulation Disorders Following Acute Head Injury

摘要 本文观察了35例闭合性颅脑损伤患者伤后24小时和伤后第七天血凝功能的变化。结果显示:伤后24小时全部患者均有不同程度的血凝障碍,以纤维蛋白分解产物(FDP)升高最常见;伤后第七天存活组除1例迟发颅内血肿外,血凝功能均有明显恢复,伤后1周后死亡者血凝障碍仍显著。本组患者临床上均未发生DIC。根据实验结果,作者认为血凝障碍等可作为衡量颅脑损伤严重程度的客观指标。血凝障碍是判断颅脑损伤患者早期预后的可靠依据。 The coagulation disorders following acute head injury were invesugated in 35 patients within 24 hours and the 7th day after head injury. The disorders were found in all patients within 24 hours. As to most survivors, the coagulution disorders were still obvious on the 7th day after head injury. The significant relation between coagulation disorders and both severity and early outcome of head injury were found. The level of FDP were closely correlated with the amount of dernaged tissues. Two pa- tients had delayed intracerebral heamotoma with remarkble coagulation disorders, suggesting that the disorders may play an important part in the occurance of delayed intracerebral heamotoma.
