慢性阻塞性肺疾病血浆胰岛素和C肽水平的检测 被引量:2

DETECTION OF PLASMA INSULIN AND C-PEPTIDE LEVELS IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE

摘要 ①目的 探讨慢性阻塞性肺疾病 (慢阻肺 )病人胰腺B细胞功能变化及对血糖代谢的影响。②方法对 1 68例慢阻肺病人在发作期及缓解期分别测定其血浆胰岛素、C肽和血糖水平 ,并与正常对照组进行比较。③结果 发作期慢阻肺组与正常对照组相比较 ,空腹血糖水平无明显差异 (t=1 .51 4 ,P >0 .0 5) ,但在餐后 60 ,1 2 0min时慢阻肺组的血糖水平明显高于对照组 (t =1 8.82 ,8.83 ,P <0 .0 1 )。正常对照组血浆胰岛素和C肽水平在餐后1 80min时已经接近空腹水平。慢阻肺病人的血浆胰岛素和C肽水平在餐后 1 2 0 ,1 80min时仍明显高于对照组(t=5 .79~ 8.80 ,P <0 .0 1 )。在缓解期两组各时相血浆胰岛素和C肽水平无显著性差异 (t=0 .963~ 1 .875 ,P >0 .0 5)。④结论 慢阻肺病人糖耐量明显降低 。 Objective\ To study the functional changes of β cell of pancreatic islet and its influence on blood sugar metabolism in pat ients with chronic obstructive pulmonary diseases. \ Methods\ Plasma insulin ,C peptide and blood sugar levels were de tected in 168 patients during the attack and remission. The results were compare d with those of the control group.\ Results During the episode, fasting blood sugar levels were n ot significantly different as compared with the controls ( t=1.514, P >0.05), but the blood sugar of the patients at 60 and 120 minutes after meals was obvio usly higher than that of the control group ( t= 18.82 , 8.83, P <0.01). For the normal control group, the peak time of the secretions of insulin and C pep tide was at 60 minutes after meals, and reached nearly fasting levels at 180 min utes. For the patients , the peak time of the secretions of insulin and C pepti de was at 120 minutes after meals, and the levels at 180 minutes were still obvi ously higher than those for the control group ( t= 5.79-8.80, P <0.01).On r emission, there was no significant difference between the levels of insulin and C peptide for both groups ( t= 0.963- 1.875, P >0.05). \ Conclusion\ The glucose tolerance and secretions of islet βcell a re impaired in patients with chronic obstructive pulmonary disease and probably the insulin resistance exists. [
