胶体金CWSAg-DIPSTICK在检测日本血吸虫病中的应用

Application of colloidal gold CWSAg-DIPSTICK in detecting schistosomiasis japonica

目的研究胶体金CWSAg-DIP-STICK在检测日本血吸虫病中的应用。方法以日本血吸虫可溶性虫卵抗原(SEAg)、尾蚴抗原(CeAg)为诊断抗原,采用胶体金CWSAg-DIP-STICK对325例血吸虫病人(其中急性血吸虫病28例,慢性血吸虫病297例),79例肝吸虫病人,157例囊虫病人,184例旋毛虫病人,165例肝炎病人,481例肺结核病人,876例普通就诊者以及518例正常人的临床应用与国内几种常用的血清学诊断方法比较试验。结果胶体金CWSAg-DIPSTICK操作时间为2～5min,阳性检出率为(97.31％～100％)。其中慢性血吸虫病97.31％(289/297),急性血吸虫病100％(28/28);交叉反应率为0.62％～4.89％,其中,肺吸虫病1.96％(3/153),肝吸虫病1.33％(1/75),囊虫病0.64％(3/153),旋毛虫病4.89％(9/184),肝炎1.21％(2/165),结核病0.62％(3/481);假阳性率为0.19％～1.25％,其中,寄生虫病特检门诊普通就诊者1.25％(11/876)、正常人0.19％(1/518)。结论胶体金日本血吸虫病快速诊断试纸具有特异性较强,操作简便,反应快速,敏感性较高,假阳性率低等优点,值得临床上推广应用。

Objective To study application of colloidal gold CWSAg-DIPSTICK in detecting Schis- tosomiasis Japonica. Methods Colloidal gold CWSAg - DIPSTICK was used to detect soluble egg antigen (SEAg), Cercaria antigen (CeAg) of Schis- tosoma Japonica, in 325 schistosomiasis (28 acute cases, 297 chronic cases), 79 fascioliasis hepatica, 157 cysticercisis, 184 trichinosis, 165 hepatitis, 481 tuberculosis, 876 general patients, 518 normal sub- jects, and compared with some used serum diagnostic assays. Results The operating time is 2 ～ 5 min- utes, positive rate is 97. 31 % ～ 100 %, (97. 31 % in chronic schistososomiasis, 100% in acute ones ). Cross - reactivity rates ranged from 0. 62% to 4. 89% (1. 96% (3/153 ) in paragonimosi, 1. 33% (1/75 ) in fascioliasis hepatica, 0. 64% (3/153 ) in cysticercosis, 4. 89 % (9/184) in trichinosis, 1. 21 % (2/165) in hepatitis, 0. 62 % (3/481 ) in tuberculo- sis), respectively. False postive rate is 0. 19% ～ 1. 25 % (1. 25 % (11/876) in patients from Parasite Special Clinic, 0. 19% (1/518 ) in normal persons). Conclusion The way to detect schistosomiasis Japonica with colloidal gold rapid diagnostic test paper has many advantages: strong specificity, perform simply, reaction quickly, high sensitivity, low false positive rate. So it should be recommended to spread application in clinic.
