经静脉自体骨髓移植治疗扩张型心肌病并心力衰竭1例

A case of autologous bone marrow transplantation for dilated cardiomyopathy and heart failure by intravenous infusion

目的 报告自体骨髓移植治疗扩张型心肌病并心力衰竭 1例。方法 患者女性,20岁,诊断为扩张型心肌病并心力衰竭,心功能Ⅱ级（NYHA分级）,自其右侧骼后上棘采集自体骨髓,骨髓细胞（单核细胞）计数1.7×109个,经静脉输注法行自体骨髓移植。结果 患者恢复日常活动,心功能I级,左室射血分数（LVEF）从术前 37％增加至自体骨髓移植一个月后的 56％和六个月后的 64％,左室收缩末径（LVESD）从术前 45mm缩小至自体骨髓移植一个月后的 36mm和六个月后的 34mm。然而左室舒张末径没有显著的变化。结论 自体骨髓移植治疗扩张型心肌病是一种安全、有效的细胞治疗法,它通过加强心肌收缩力改善患者心功能。

Objective To report a case of autolo-gous bone marrow transplantation (ABMT) for dilated cardiomyopathy (DCM) and heart failure (HF). Methods A female patient, aged 20 years, with diagnosis of DCM and HF (Grade II of cardiac function, NYHA) underwent ABMT at our institute. Autologous bone marrow was collected from her right posterior superior iliac spine; The number of bone marrow cells (MNCs) was 1.7×109; Cells were transplanted by intravenous infusion. Results This patient resumed daily work and her cardiac function recovered to grade I, LVEF increased obviously from 37% to 56% and 64%, LVESD decreased from 45mm to 36mm and 34mm after one month and six months, respectively. However, there was no distinct change in her LVEDD. Conclusion ABMT for DCM is a safe and effective cell therapy, it can improve cardiac function by reinforcing its contraction.
