指背逆行岛状皮瓣修复指端缺损及晚期功能评价

Repair of Distal Finger Defect with the Dorsal Digital Retrograde Island Skin Flap and Their Late Stage Functional Evaluation

目的 :探讨修复指端的理想方法——指背逆行岛状皮瓣。方法 :以指固有动脉背侧支为血管蒂 ,手指近节指背近 1 /3以远的皮瓣 ,修复同指的指端缺损 ,皮瓣最小 1 .5 cm× 1 .0 cm,最大面积2 .5 cm× 2 .0 cm。结果 :临床应用 2 4 6例 2 83指均成活 ,并进行 2年以上随访 ,对其晚期功能从肤色、外观、出汗、质地、温度、植皮使用情况、痛觉、触觉、两点分辨觉、持物稳定情况 1 0项指标进行综合评定。优 80～ 1 0 0分 ,良 60～ 79分 ,可 40～ 5 9分 ,差 <40分。优良率 95 .3%。结论 :该方法具有手术方法简单、安全 ,血管蒂长 ,旋转弧度大 ,不损伤指固有动脉 ,且晚期功能良好等优点。是一种修复指端缺损的理想方法。

Objective: To explore the dorsal digital retrograde island skin flap in treating distal finger skin defect.Methods:Retrograde island skin flap was designed pedicled with the digital artery proper of the finger. The flap area was 1.5 cm×1.0 cm to 2.5 cm to 2.0 cm.Results:283 fingers (246 cases) were treated with suck skin flap with 100% success. Evaluation of the finger function was carried out 2 years later to examine the finger color, appearance, substance, sweating, warmness, pain feeling, sense of touch and distance recognition. The best score was 80 to 100, good score was 60 to 79, average score was 40 to 59, bad score was below 40. The evaluation showed 95.3% best score.Conclusion:The retrograde island skin flap was effective in treating distal finger skin defect with good functional restoration.
