90株肠杆菌属细菌诱导型β-内酰胺酶的检测与药敏结果

摘要 目的 :了解肠杆菌属细菌产诱导型 β-内酰胺酶及药敏情况。方法 :应用双纸片扩散法——分别采用亚胺培南和头孢西丁两种诱导剂以及头孢他啶和头孢噻肟两种目标物——测定 90株肠杆菌属细菌的诱导型β-内酰胺酶 ,并用 VITEK3 2 -AMS测定该 90株细菌对 14种抗生素的耐药性。结果 :亚胺培南组和头孢西丁组分别有 5 1株和 2 5株细菌产生诱导型 β-内酰胺酶 ,头孢他啶组和头孢噻肟组分别有 5 0株和3 0株细菌的诱导型β-内酰胺酶阳性 ,以头孢西丁、头孢噻吩、氨苄西林的耐药率最高 ,亚胺培南的耐药率最低。结论 :亚胺培南优于头孢西丁、头孢他啶优于头孢噻肟 ,5 1株产酶组与 3 9株非产酶组细菌的耐药差异无显著性。临床用药时应注意亚胺培南的高诱导性和体外药敏试验的局限性 ,提倡微生物实验室注重诱导型 Objective:To study the production of inducible Beta Lactamase in Enterobacter spp and the station of antibiotic susceptibility.Methods:Detected the inducible Beta Lactamase in 90 strains Enterobacter spp by double disks diffusion test,which used Imipenem and Cefoxitin as the revulsants and used Ceftazidime and Cefotaxime as substrate.Meanwhile we detected the susceptibility of the above mentioned 90 strains Enterobacter spp against 14 kinds of antibiotic on VITEK32 AMS.Results:51 strains(56 7%) bacteria were positive when Imipenem as inducer and 25 strains (27 8%) were positive when Cefoxitin as inducer,50 strains (55 6%) bacteria were positive when Ceftazidime as substrate and 30 strains (33 3%) were positive when Cefotaxime as substrate. It conclude that Imipenem is stronger than Cefoxitin and Ceftazidime is stronger than Cefotaxime,and there is no obvious difference between the 51 strains bacteria group of producing enzyme and the 39 strains bacteria group of non producing enzyme.Conclusion:The docters use to pay attention to high inductivity of Imipenem and localization of the antibiotic susceptibility test in vitro .,advocate microorganism laboratory to think much of measurement of inducible Beta Lactamase.
