腺样囊性癌E-cad、LN、FN和PCNA表达的临床意义

Adenoid cystic carcinoma: clinical significance of expression of E-cadherin, laminin, fibronectin and proliferating cell nuclear antigen

目的 探讨腺样囊性癌 (ACC)上皮细胞黏附分子钙黏蛋白 (E cad)、层黏连蛋白 (LN)、纤维黏连蛋白 (FN)和增殖细胞核抗原 (PCNA)表达与预后的关系。方法 应用免疫组化方法对 34例腺样囊性癌E cad、LN、FN和PCNA的表达进行检测。结果 E cad低表达率为 5 0 % (17/ 34) ,E cad低表达与ACC组织类型、临床分期、复发 /转移及 5年生存率显著相关 (P <0 0 5 )。实体型、临床Ⅲ～Ⅳ期、有复发 /转移及术后生存期 <5年的ACC ,基底膜缺损程度显著高于筛状管状型、Ⅰ～Ⅱ期、无复发 /转移及术后生存期 >5年者 (P <0 0 5 )。PCNA计数标记指数平均为30 9% ,前者中PCNA指数均显著增高 (P <0 0 1)。结论 E cad、LN、PCNA的表达可作为临床判断ACC恶性程度、评估预后的有价值指标。

Objective To investigate the relationships between the expression of cell adhesion molecules E cadherin (E cad),laminin (LN),fibronectin (FN)and proliferating cell nuclear antgen (PCNA)and prognosis in adenoid cystic carcinoma (ACC) Methods The expression of E cad, LN, FN and PCNA in 34 cases of ACC were detected by immunohistochemical staining Results E cad expression was absent or low in 50% of ACC Absent or low E cad expression were significantly related to the histological types, clinical stages, recurrence and/or metastasis and 5 year survival rate of ACC (P<0 05) Heavily defective basement membrane was observed more frequently in ACC of solid type, clinical Ⅲ～Ⅳ stages, recurrence and/or metastasis and with less than 5 year survival period (P<0 05) PCNA labeling index (average,30 9%) was significantly high in ACC of solid type, clinical Ⅲ～Ⅳ stages, recurrence and/or metastasis and with less than 5 year survival period (P<0 01) Conclusions Expression of E cad, LN and PCNA can be regarded as valuable indicators for evaluating malignant degree and the prognosis of ACC.
