摘要
目的 :探讨颅内星形细胞瘤MRI增强前后的一些定量指标在鉴别诊断中的作用。材料和方法 :对星形细胞瘤 3 8例、脑膜瘤 2 0例、血管母细胞瘤 12例、转移瘤 16例、听神经瘤 10例、肉芽肿性病变 10例 ,增强前后的MRI图像测量其增强比值 (% )、平扫肿瘤相对信号强度 (RSIc )及增强后肿瘤相对信号强度 (RSIGd)几个指标 ,并对这几个指标在各组肿瘤中进行了统计和分析。结果 :RSIGd在低分化与高分化星形细胞瘤之间及与其它各组之间均有差异 ,低分化星形细胞瘤与听神经瘤和血管母细胞瘤相比差异更显著 ,RSIc 在各组中的数值较集中 ,各组之间无明显的统计学意义。结论 :RSLGd不论是在星形细胞瘤分化的判断、还是在与其他脑内肿瘤的鉴别诊断中都是一个敏感的指标 ,增强比值在鉴别诊断中也有一定的价值。
Purpose: To evaluate the valuable parameters in the diagnosis and differential diagnosis of the brain astrocytoma by means of plain and contrast enhanced MRI scan. Materials and methods: Enhancement ratio (%), relative signal intensity of the tumor and the white matte in plain scan (RSIc-) and in contrast scan (RSI Gd) were obtained after analyzing the pre- and post contrast enhanced MR imaging of 96 cases of intrascranial tumors, including which astrocytoma (n=38) Acoustic neuroma (n=10),meningioma (n=20) and angioblastoma (n=12) and metastatic tumor (n=16) and granuloma (n=10). Results: RSI Gd was different between low and high differentiated degree astrocytoma (P<0.05), and it is markedly different between low differentiated degree astrocytoma group and other tumor groups (P<0.01). RSIc- was not statistical significance between all groups. Conclusion: RSI Gd sensitively reflects the differentiated degree of astrocytoma and it is helpful for differential diagnosis of brain tumors. Enhancement ratio is also helpful for dfferential diagnosis in some occasions.
