The effect of occlusal reduction on pain after endodontic instrumentation

摘要 目的 :探讨降低咬合对牙髓治疗后疼痛的影响。方法 :将患牙 10 5颗分为全面降低咬合组 (1组 )、模拟降低咬合组 (2组 )和对照组 (3组 ) ,分析它们在有无牙髓活力、根周X线透射区、叩痛或术前疼痛等情形下的术后疼痛反应。结果 :对于活髓、无根周X线透射区、有叩痛、有术前疼痛的患牙 ,1组与 2组、3组之间的疼痛发生率有显著差异 ,而其余相反 4种情形下 ,3组之间的疼痛发生率无显著差异。结论 Objective: To observe the effect of occlusal reduction on pain after endodontic instrumentation. Methods: 105 posterior teeth in patients were divided into three groups: (1) total occlusal reduction, (2) simulated occlusal reduction or (3) control. The pain response with or without vital pulp, periadicular radiolucency, percussion sensitivity or preoperative pain were recorded and analyzed. Results: In group (1) the patients with vital pulp, presence of percussion sensitivity, absence of a periadicular radiolucency, and presence of preoperative pain postoperative pain response was significantly decreased by reduction of total occlusion ( P >0.01). Conclusion: Occlusal reduction might diminish the rate of postoperative pain following endodontic instrumentation.
