显微根尖手术联合iRoot BP Plus治疗难治性根尖周炎的效果

Effect of microsurgery combined with iRoot BP Plus on refractory periapical periodontitis

目的研究显微根尖手术与iRoot BP Plus方案联合治疗难治性根尖周炎患者的效果。方法选择2015年6月至2018年8月诊治的62例难治性根尖周炎患者为回顾分析对象,随机分为两组,每组31例。观察组患者以显微根尖手术配合iRoot BP Plus治疗,对照组患者以显微根尖手术配合MTA治疗。结果观察组总有效率为96.77%(30/31),对照组为93.55%(29/31),差异未见统计学意义(P>0.05)。观察组患者术后1周肿痛程度明显低于对照组(P<0.05)。两组患者术前旧根尖周指数(O-PAI)比较,差异未见统计学意义(P>0.05);术后3个月和6个月观察组患者的新根尖周指数(N-PAI)改善效果好于对照组(P<0.05)。结论对于难治性根尖周炎患者以显微根尖手术配合iRoot BP Plus治疗有助于提升临床疗效,患者术后发生肿胀风险较低,有助于促进根尖周组织的恢复,该治疗方案值得临床应用和推广。

Objective To investigate the clinical efficacy of combined microscopic apical surgery and iRoot BP Plus on refractory periapical periodontitis.Methods Sixty-two patients with refractory periapical periodontitis from June 2015 to August 2018 were selected for retrospective analysis.They were randomly divided into two groups,with 31 cases in each group.The patients in the observation group were treated with microscopic apical surgery combined with iRoot BP Plus,while the patients in the control group were treated with microscopic apical surgery combined with MTA.Results The total effective rate of treatment in the observation group was 96.77%,which was slightly higher than that in the control group(93.55%),but there was no significant difference between the two groups(P>0.05).The degree of swelling and pain in the observation group was significantly lower than that in the control group at 1 week after operation(P<0.05).There was no significant difference in preoperative old-periapical index(O-PAI)between the two groups(P>0.05).The improvement effect of new-periapical index(N-PAI)in the observation group was better than that in the control group at 3 and 6 months after surgery(P<0.05).Conclusions For patients with refractory periapical periodontitis,microscopic apical surgery combined with iRoot BP Plus is helpful to improve the clinical efficacy.The risk of postoperative swelling in patients is low,which is helpful to promote the recovery of periapical tissues.This treatment scheme is worthy of clinical application and promotion.
