清化益胃法干预幽门螺旋杆菌家庭聚集性感染的临床研究

Clinical study of Qinghua Yiwei method on intervention of helicobacter pylori family clustering infection

目的研究清化益胃法对干预幽门螺旋杆菌(Helicobacter pylori,HP)家庭聚集性感染的临床应用。方法将2015年1月至2016年12月150例Hp感染患者按区划随机法分为对照组、观察组A、观察组B,每组50例。对照组采用铋剂四联方案治疗,观察组则以清化益胃法治疗,其中观察组A疗程为2周,观察组B疗程为4周,对比三组临床症状、胃镜下黏膜表现、Hp复感率及家庭成员阳性率、药物不良反应症状。结果观察组B总有效率为96.00%(48/50),显著高于对照组及观察组A(P<0.05)。治疗后观察组A、观察组B胃镜下黏膜分级明显改善(U=5.543、20.151,P<0.05),且观察组B改善幅度显著优于对照组(U=12.225,P<0.05)。同时,观察组BHP复感率及观察组A、观察组B家庭成员阳性率均显著低于对照组(P<0.05)。结论清化益胃法干预HP家庭聚集性感染可降低复感率及家庭成员阳性率,对于临床症状、胃镜下黏膜表现的改善均有重要意义,且安全性良好。

Objective To study the clinical application of Qinghua Yiwei method on intervention of helicobacter pylori(Hp)family clustering infection.Methods From January 2015 to December 2016,150 cases of H.pylori infection were randomly divided into control group,observation group A and observation group B,with 50 cases in each group.The control group was treated with Bismuth quadruple therapy,while the observation group was treated with Qinghua Yiwei method.The treatment course of observation group A was 2 weeks and that of observation group B was 4 weeks.The clinical symptoms,gastroscopic mucosal manifestations,H.pylori relapse rate,positive rate of family members and adverse drug reactions were compared among the three groups.Results The total effective rate of observation group B was 96.00%,which was significantly higher than that of control group and observation group A(P<0.05).After treatment,the gastroscopic mucosal grading of observation group A and B was significantly improved(U=5.543,20.151,P<0.05),and the improvement of observation group B was significantly better than that of control group(U=12.225,P<0.05).Meanwhile,the rate of H.pylori relapse in observation group B and the positive rate of family members in observation group A and B were significantly lower than those in control group(P<0.05).Conclusions The intervention of Qinghua Yiwei method in H.pylori family cluster infection can reduce the rate of relapse and positive rate of family members.It is significant for improving clinical symptoms and gastroscopic mucosal manifestations,and has good safety.
