摘要
目的 探讨高强度聚焦超声 (highintensityfocusedultrasound ,HIFU)对恶性肿瘤局部组织中免疫状态的影响。方法 利用Wistar大鼠 ,采用癌组织块包埋法建立大鼠W2 5 6肝癌模型 (4 0只 ) ;同时 ,以相同程序建立正常肝对照组 (C组 ,大鼠 2 0只 )。 10d后 ,应用HIFU(10MHz,15 2 8 7W/cm2 ,2 0s× 2次 )对治疗组 (A组 ) 2 0只大鼠的癌灶进行照射 ;以同样方式照射C组大鼠相同部位的正常肝组织 ;荷癌肝对照组 (B组 )则按同样程序对 2 0只大鼠的癌灶进行假照处理。 2周后活杀动物取材。进行组织学观察 ,并用免疫组织化学方法对照射区周围局部组织中的T淋巴细胞亚群 (T lympho cytesubsets ,T LS)进行原位半定量观察。结果 光学显微镜下见靶区组织凝固坏死 ,无癌细胞或肝细胞残存。A组局部组织中CD4 +和CD8+细胞数量 (16 1± 6 3 ,12 0± 5 2 )个 ,较B组 (5 8± 3 0 ,8 3±4 7)个、C组 (1 7± 1 1,1 2± 0 6 )个显著增加 (P <0 0 0 1,P <0 0 5 ;P <0 0 0 1,P <0 0 0 1)。其中尤以CD4 +细胞增加明显 ,CD4 +/CD8+细胞比值大于 1;B组的CD4 +和CD8+细胞数量 (5 8± 3 0 ,8 3± 4 7)个亦明显高于C组的 (1 7± 1 1,1 2± 0 6 )个 (P <0 0 0 1,P <0 0 0 1)。但CD4 +/CD8+细胞比值 (0 7±0 3)小于 1;C?
Objective[WT5”BZ] To investigate the effect of high intensity focused ultrasound (HIFU) on the local immunity of the stroma surrounding the tumor.[WT5”HZ]Methods[WT5”BZ] Forty Wistar rats were used to establish tumor models by implanting W256 tumor (size:1*!mm 3) into their left liver,half of which were allocated into group A,and the other half into group B with sham treatment.20 control rats into group C. Ten days later,tumors in group A were irradiated by HIFU with a frequency of 10*!MHz, intensity of 1*!528 7*!W/cm 2, 20 seconds per time (2 times). The normal liver tissues of group C were treated as group A. Tumors of group B were used as control of sham irradiation. All rats were sacrificed 2 weeks after and specimens were taken.The histological changes of the target lesions were observed, and immunohistological chemistry method was employed to semiquantatively assess the T-lymphocyte subsets(T-LS)in the stroma surrounding the lesion.[WT5”HZ]Results[WT5”BZ] Light microscopy revealed that the target lesions were of coagulative necrosis without tumor cells or hepatic cells surviving. There were much more CD 4 + and CD 8 + cells in the stroma of group A(16 1±6 3, 12 0±5 2) than those of the control groups.Especially CD 4 + cells increased significantly. CD 4 +/CD 8 + ratios (1 4±0 5) were similar to that of group C (1 5±0 7)(>1∶1) but larger than that of group B(0 7±0 3) (<1∶1). Nevertheless, the numbers of CD 4 + and CD 8 + cells of group B (5 8±3 0, 8 3±4 7) were larger than those of group C (1 7±1 1, 1 2±0 6).[WT5”HZ]Conclusions[WT5”BZ] Tumor could induce a special immune reaction. HIFU could improve and correct the abnormal status of the host immunity, and enhance the local immunity against tumor. [WT5”HZ]
