CT诊断成人胆管囊肿合并胆道系统肿瘤

The CT diagnosis of choledochal cyst complicated by biliary tract carcinoma

目的 提高对胆管囊肿合并胆道系统肿瘤病例的认识。方法 回顾性分析经病理学、临床和多种影像学检查证实的 10例临床资料。结果 胆管囊肿Ⅰ型 6例 ,Ⅳ型 2例 ,Ⅴ型 2例 ;胆管囊肿合并胆管癌 7例 (7/ 10 ) ,合并胆囊癌 2例 ,合并胰腺癌 1例。 7例胆管癌以肿块为主要表现者 3例 ,胆管壁增厚 4例 ;出现特征性延迟相强化者 6例 (6 / 7)。CT诊断胆管囊肿合并胆道系统肿瘤的准确性为 90 % (9/ 10 )。结论 成人胆管囊肿合并胆道系统肿瘤是少见疾病 ,由于成人胆管囊肿并发胆道肿瘤的机会较高 ,在行CT检查时要高度警惕。CT在该病的诊断和鉴别诊断中有很高的准确性。

Objective[WT5”BZ] To futher look into the likelyhood and occurrence of carcinoma arising from choledochal cyst.[WT5”HZ]Methods[WT5”BZ] Ten cases of choledochal cyst associated with bile duct carcinoma were reviewed, the CT appearance and imaging feature were evaluated.[WT5”HZ] Results[WT5”BZ] In our series, cancer arising from type Ⅰ cyst in 6 cases (60 0%),type Ⅳ in 2,and type Ⅴ in 2. The CT feature of specific delayed enhancement was seen in 6 out of 7 cases of choledochal carcinoma, deffuse irregular thickening of the wall (>5 mm) in 4 cases, and polypoid mass in 3.[WT5”HZ] Conclusion[WT5”BZ] CT is a new non invasive technique, evaluable in the diagnosis of chololedochal cyst complicated with carcinoma.
