摘要 目的 评价感觉神经端侧吻合后再生神经的功能。方法 取兔耳大神经移植体作为受神经 ,与对侧耳大神经端侧吻合并植入皮瓣。用神经单纤维放电引导电生理技术检测皮瓣内再生放电纤维数量、分布、感觉类型。同时以兔 2条耳大神经端侧吻合模型 ,用RM 6 2 80多道智能生理记录及分析处理系统测定再生纤维动作电位幅值恢复率和神经传导速度。结果 植入皮瓣的受神经在术后 4个月可再生触、压、痛、温冷觉末梢 ,使皮瓣初步建立神经再支配。受神经内再生纤维的动作电位幅值恢复率及神经传导速度随时间延长逐步增加 ,术后 30周受神经近端再生纤维幅值恢复率和传导速度分别达到正常的 88 8%和 94 38%。结论 感觉神经端侧吻合后能再生有功能的神经纤维 。 Objective To estimate the function of regen erating nerve after sensory nerve end to side neurorrhaphy Methods One of the great auricular nerve graft of a rabbit was taken as the receptive nerve and was anastomosed to the lateral side of the other great auricular nerve of the same rabbit and then implanted into the flap The amount distribution and types of sensation of regenerating nerve fibers were examined by measurement of single nerve fiber induced discharge At the same time, the recovery rate of action potential and nerve conduction velocity of regenerating nerve were studied using RM 6280 multi tract intelligent recording and analyzing system of physical signals in end to side neurorrhaphy animal model consisting of a rabbit's two greater auricular nerves Results The receptive nerve which implanted into the f lap can regenerate nerve endings capable of producing tactile, pressure, pain and thermo senses and re innervate the flap The recovery rate of action potential and nerve conduction velocity of the regenerating nerve fibers in receptive nerve increased with time elongation 30 weeks after the operation, the recovery rate of action potential and nerve conduction velocity of the regenerating nerve fibers in proximal portion of receptive nerve were 88 8% and 94 38% respectively Conclusion Sensory nerve can regenerate functional nerve fibe rs after end to side neurorrhaphy, but the process is rather slow
