^(18)氟-脱氧葡萄糖正电子发射计算机断层显像在探测乳腺癌复发和转移中的应用

摘要 目的 对 30例乳腺癌术后患者进行18氟 脱氧葡萄糖 (18F DG)正电子发射计算机断层显像 (PET)检查 ,评价其在探测乳腺癌复发和转移中的应用价值。方法  30例经治疗后临床疑有肿瘤复发或转移的乳腺癌患者均进行FDGPET全身显像 ,应用目测法和半定量分析法判断结果 (标准摄取值 ,SUV)。分别对每例患者及其病灶进行分析 ,比较FDGPET与常规影像学检查对病灶检出率的不同。结果  30例患者中 ,12例PET检查未见异常 ,18例可见局部复发和 /或转移。 1例骨转移患者PET为阴性。FDGPET检测灵敏度为 94 .7% ,常规影像学检查灵敏度为 73.7%。在 12 2个肿瘤转移灶中 ,PET检出 116个 (95 % ) ,常规影像学检查检出 84个(6 9% ) ,FDGPET对病灶的检出率明显高于常规影像学检查 (P <0 .0 0 1)。结论 FDGPET全身显像有助于早期诊断乳腺癌治疗后的肿瘤复发和 Objective To evaluate the value of whole body 18 F DG PET imaging for the detection of recurrent or metastatic breast cancer after treatment. Methods Whole body 18 F DG PET imaging with attenuation correction was performed in 30 patients with suspected recurrent or metastatic breast carcinoma. PET images were evaluated for each patient and lesion with comparison of correctional imaging outcomes. Results Out of the 30 patients, 12 had normal PET scan, 18 had local recurrence or distant metastases. One patient with bone metastases showed negative FDG PET. The sensitivity was 94.7% for 18 F DG PET scans and 73.7% for conventional imaging. 18 F DG PET scans identified 116 out of 122 lesions(95%) with recurrent or metastatic disease and conventional imaging identified only 84 lesional sites(69%). The detection rate of recurrent or metastatic breast cancer for 18 F DG PET was significantly higher than that of conventional imaging( P <0.001). Conclusion The preliminary study indicates that whole body 18 F DG PET scan is very useful for detecting recurrent or metastatic breast carcinoma in patients suspected of having recurrent disease post therapy.
