摘要
目的探讨血清高同型半胱氨酸(Hcy)、β2微球蛋白(β2MG)、超敏C反应蛋白(hsCRP)检测在H型高血压冠心病患者中的意义。方法选取遵义市第一人民医院心血管内科2014年10月至2015年12月收治的140例原发性高血压伴冠心病的患者作为研究对象,其中62例患者血Hcy≥10μmol/L(H型高血压组)、78例患者血Hcy<10μmol/L(非H型高血压组),对比两组患者血Hcy、β2MG及hs-CRP的水平,并分析其与冠状动脉病变程度的关系。结果 H型高血压组患者的Hcy、β2MG、hs-CRP及Gensini评分均显著高于非H型高血压组(P<0.05)。H型高血压组患者中的Hcy、β2MG、hs-CRP及Gensini评分在冠状动脉单支病变、双支病变、三支病变患者中呈递增趋势(P<0.05);H型高血压组患者中的Hcy、β2MG、hs-CRP及Gensini评分在稳定性心绞痛组、不稳定心绞痛组、急性心肌梗死组中呈递增趋势(P<0.05);H型高血压组患者中的Hcy、β2MG、hs-CRP与Gensini评分均显著正相关(P<0.05)。结论 H型高血压冠心病患者血Hcy、β2MG及hs-CRP与患者冠状动脉病变程度具有紧密的相关性。
Objective To investigate the significance of serum Hcy, β2MG and hs-CRP in patients with H typehypertension and coronary heart disease. Methods 140 patients with essential hypertension and coronary heart diseasein the First People's Hospital of Zunyi from October 2014 to December 2015 were selected, including 62 patientswith Hcy≥10μmol/L (H type hypertension group) and 78 patients with Hcy<10μmol/L (non H type hypertensiongroup). The serum levels of Hcy, β2MG and hs-CRP were compared between the two groups. The relationshipbetween the degree of coronary artery disease with serum Hcy, β2MG and hs-CRP were analyzed. Results Hcy,β2MG, hs-CRP and Gensini scores in patients with H type hypertension were significantly higher than those in thenon H type hypertension group (P<0.05). Hcy, β2MG, hs-CRP, Gensini scores in H type hypertensive patients withcoronary single-vessel disease, or double vessel disease, or triple vessel disease were gradually increased (P<0.05).Hcy, β2MG, hs-CRP and Gensini score in H type hypertensive patients with stable angina, unstable angina, or acutemyocardial infarction group were gradually increased (P<0.05). Hcy, β2MG, and hs-CRP were significantly positivelycorrelated with Gensini scores in patients with H type hypertension (P<0.05). Conclusion Serum Hcy, β2MG and hs-CRP were correlated with the degree of coronary artery disease in patients with H type hypertension.
