大容量扩张器全扩张法耳廓再造术 被引量:8

摘要 目的:探讨大容量皮肤扩张器加自体肋软骨移植耳廓再造的手术方式及临床效果。方法:53例先天小耳畸形患者I期使用应用皮肤软组织扩张3个月,二期自体肋软骨雕刻为耳支架,无需植皮,行耳廓再造术治疗。结果:53例随访6个月至2年,47例再造耳廓位置基本正常。皮肤质地较好,三维结构清晰;1例耳廓塌陷,2例无良好耳颅角,3例皮肤破溃。结论:皮肤扩张器全扩张法加自体肋软骨移植耳廓再造术是较为简单有效且并发症少的耳再造方式。 Objective To investigate a method of atuologous cartilage auricular reconstruction by large volume skin expansiontechnique. Methods 53 cases microtia was operated by this method. Skin soft- tissue expander was implanted during stageI,and skin was expended for 3 months. During stage Ⅱ,the autologous rib cartilage framework was made and implanted intothe expanded skin without skin free graft. Results A follow-up of 6 to 12 months was carried out for these 53 patients, goodcosmetic results were got in 47 cases of the reconstructed ear. Cartilage-framework was collapsed in 1 case, and no good otocranialangle, and necrosis of skin happened in 3 cases. Conclusion Auricular reconstruction by autologous rib-cartilage withbig volume skin expansion technique was a simple and effective method and with less complications.
