高血压伴阵发性房颤患者的心脏超声临床特点分析

Analysis of clinical features of cardiac ultrasound in patients with hypertension and paroxysmal atrial fibrillation

目的:研究高血压伴阵发性房颤患者的心脏超声临床特点。方法:选取我院2014年4月—2016年1月收治的88例高血压伴阵发性房颤患者,作为研究组;选择同期88例单纯高血压疾病患者作为对比组。两组患者均给予飞利浦IE33超声仪进行心脏超声检查,观察指标:患者左房内径、左室舒张末期内径、舒张早期心室快速充盈峰值速度/舒张晚期心房收缩峰值速度(E/A比值)、室间隔/左室后壁的差异。结果:研究组左房内径为(44.89±3.13)mm,明显比对比组(32.81±2.31)mm高,t检验统计学差异显著(t=12.824,P<0.01);研究组左室舒张末期内径为(43.82±12.42)mm,跟对比组(41.23±12.12)mm比较,t检验统计学差异不显著(t=0.342,P>0.05);研究组E/A比值为(0.82±0.21),明显比对比组(0.61±0.12)高,t检验统计学差异显著(t=5.924,P<0.01);研究组室间隔/左室后壁为(1.12±0.13),跟对比组(1.09±0.12)比较,t检验统计学差异不显著(t=0.241,P>0.05)。结论:高血压伴阵发性房颤患者的心脏超声临床特点为左房内径明显增大,E/A比值增大,说明高血压伴阵发性房颤跟左心房扩大有密切关系,可因血压升高而导致心脏结构发生变化,引起形态学重构。

Objective: To study the clinical features of cardiac ultrasound in patients with hypertension and paroxysmal atrialfibrillation. Methods: Experimental objects: 88 patients with hypertension and paroxysmal atrial fibrillation, admitted to our hospital fromApril 2014 to January 2016, were selected as the research group; 88 patients with hypertension during the same period were selected as thecontrol group, patients in both groups received cardiac ultrasound by using PHILPS IE33 ultrasonic instrument. Observation indexes: thedifference of the patients’ left atrial diameter, left ventricular end diastolic diameter, peak velocity of early diastolic ventricular filling/ latediastolic peak atrial systolic velocity (E/A ratio) and interventricular septum/posterior left ventricle. Results: The left atrial diameter of theresearch group was (44.89±3.13) mm, which was significantly higher than that of the control group, (32.81±2.31) mm, the difference wasstatistically significant by t test (t=12.824, P<0.01). The left ventricular end diastolic diameter of the research group was (43.82±12.42)mm, and that of the control group was (41.23±12.12) mm, the difference was statistically insignificant by t test (t=0.342, P>0.05). TheE/A ratio of the research group was (0.82±0.21), which was significantly higher than that of the control group, 0.61±0.12, the differencewas statistically significant by t test (t=5.924, P<0.01). The interventricular septum/posterior left ventricle of the research group was (1.12±0.13), and that of the control group was (1.09±0.12), the difference was statistically insignificant by t test (t=0.241, P>0.05).Conclusion: The clinical features of cardiac ultrasound in patients with hypertension and paroxysmal atrial fibrillation included thesignificant increasing of the patients’ left atrial diameter and the E/A ratio, which indicated that the hypertension and paroxysmal atrialfibrillation was closely related to left atrial dilatation, the increasing blood pressure could lead to cardiac structure changes and causemorphological reconstruction.
