真菌性角膜溃疡的临床实验诊断

Clinical laboratory diagnosis of fungal corneal ulcer

目的:提高临床上对于真菌性角膜溃疡的认识,做好鉴别诊断。方法:进行角膜刮片做10%KOH湿片直接镜检,再做沙氏琼脂平板培养后,根据菌落的生长特点,培养后湿片镜检,"回形针"法小培养特征,药敏纸片琼脂扩散法结果及抑菌环大小等进行分析。结果:镰刀菌最多46%,曲霉菌22%,青霉菌13%,链互隔菌10%,还有不常见的头孢子菌、无孢菌,不同菌种其药敏试验结果不相同。结论:真菌性角膜溃疡的早期确诊、正确用药对治疗至关重要。

Objective:To improve the clinical understanding of fungal corneal ulcer,do a good job in differential diagnosis.Methods:Do 10% KOH wet film direct microscopic examination with corneal scrapings,and then do the Sabouraud agar plate culture,wet sheet microscopy after incubation according to the growth characteristics of the colonies, “Paper clip” method of small culture characteristics,the results of susceptibility disk diffusion method and bacteriostatic ring size were analyzed.Results:Fusarium occupies 46% of the total,22% Aspergillus,Penicillium 13% ,alternaria fungus chain 10% ,there were uncommon bacteria genus such as cephalosporium,non-sporulating mycelia,the drug sensitive test result of different strains are not the same.Conclusion:Early diagnosis of fungal corneal ulcer and proper medication are essential for the treatment.
