Experience sharing of general practitioner team health management model for patients with blood pressure in community to reach target blood pressure

目的:通过全科医生团队健康管理模式对社区高血压患者血压达标经验分享,探讨社区高血压管理的最佳方案。方法:收治原发性高血压患者120例,采用全科医生团队健康管理模式对患者血压进行管理。结果:管理后患者生活方式和自我管理情况都有明显改善。收缩压与舒张压管理后与管理前比较,差异具有统计学意义(P<0.05)。不同治疗方案都能取得良好的治疗效果。结论:全科医生团队健康管理模式对社区高血压患者进行管理值得推广。

Objective:To explore the best program of community hypertension management through the experience sharing of general practitioner team health management model for patients with blood pressure in community to reach target blood pressure.Methods:120 patients with essential hypertension were selected.We used the general practitioner team health management model for managing the patients’ blood pressure.Results:After management,the life style patients and self management situation had improved significantly.The differences of systolic blood pressure and diastolic blood pressure after management were statistically significant compared with before the management(P<0.05).Different treatment could obtain good therapeutic effect.Conclusion:General practitioner team health management model for patients with blood pressure in community is worth promoting.
