基层剖宫产率增高原因及对策

The reasons and countermeasures of the increase in the rate of cesarean section at the grass roots

目的:探讨基层剖宫产率增高的原因,提出相关对策,为降低剖宫产率提供可靠依据。方法:对剖宫产病例进行回顾性分析。结果:在1 631例孕妇中,剖宫产581例,剖宫产率35.62%。从2012-2014年,剖宫产率逐年上升。剖宫产的指征以胎儿宫内窘迫为首位。城市户籍产妇多于农村户籍,初产妇多于经产妇。结论:孕产妇的围产期保健、宣传教育以及医疗纠纷、医护服务能力等因素,导致剖宫产率逐年上升,通过加强围产期保健,减少围产期并发症,正确处理产程,医患双方共同努力,才能降低基层剖宫产率。

Objective:To explore the reasons for the increase in the rate of cesarean section at the grass roots,and to put forwardrelevant measures,to provide a reliable basis for reducing the rate of cesarean section.Methods:Cesarean section cases wereretrospectively analyzed.Results:In 1 631 cases of pregnant women,581 cases of cesarean had cesarean section,and the rate ofcesarean section was 35.62%.From 2012 to 2014,the rate of cesarean section was increasing year by year.Fetal distress was themost important factor leading to cesarean section.Maternal with urban household registration were more than the rural householdregistration,and the primiparas were more than the multiparas.Conclusion:Perinatal health care,education of maternal and medicaldisputes,medical services and other factors led to the increase of cesarean section rate year by year,by strengthening perinatalhealth care,reducing perinatal complications,correctly handling the labor process,and strengthening joint efforts of both doctorsand patients can reduce the cesarean section rate at the grass roots.
