摘要
目的:探讨脐动脉血气分析p H值在新生儿窒息诊断及预后判断的应用及临床意义。方法:收治单胎足月新生儿126例,在胎儿娩出第一声啼哭后立即行脐动脉血气分析、Apgar评分。结果:新生儿脐动脉血PO2、p H、BE值等与新生儿Apgar评分呈正相关(P<0.05),PCO2与Apgar评分呈负相关(P<0.05);重度窒息组新生儿脐动脉血p H<7.20与p H>7.25患儿例数分别与轻度窒息组和正常组间差异存在统计学意义(P<0.05,P<0.01)。结论:临床诊断分娩过程中的新生儿窒息采用脐动脉血气分析能弥补Apgar评分的不足,对新生儿窒息的早期诊断与预后判断具有重要的临床意义。
Objective:To explore the application of umbilical artery blood gas analysis pH value in the diagnosis and prognosis ofneonatal asphyxia and its clinical significance.Methods:126 singleton full-term newborns were selected.All of them were takenumbilical arterial blood gas analysis and apgar score immediately as the delivery of fetus first cry.Results:Neonatal umbilicalartery blood PO2,pH and BE have positively correlated with neonatal Apgar score(P<0.05);PCO2 and Apgar score has negativelycorrelation(P<0.05);children with neonatal umbilical artery blood pH<7.20 and pH>7.25 cases in severe asphyxia group weresignificant difference with the mild asphyxia group and the normal group (P<0.05,P<0.01).Conclusion:Umbilical artery bloodgas analysis in the diagnosis of asphyxia neonatorum during the delivery process can make up deficiency for Apgar score.It hasimportant clinical significance for early diagnosis and prognosis of neonatal asphyxia.
