摘要
目的:探讨糖尿病自我管理学校的社区糖尿病管理。方法:成立糖尿病自我管理学校,采取多种形式,开展糖尿病管理和培训。结果:糖尿病患者管理数、糖尿病管理率、规范管理率、血糖控制率均有明显提升。结论:通过在社区卫生服务中心建立糖尿病自我管理学校,加强全科医师团队的慢病管理知识培训,并从患者中培养、遴选出社区志愿者,为全辖区的糖尿病患者提供同伴支持服务,使糖尿病社区管理取得了较好成效。
Objective:To discuss the community diabetes management of diabetes self-management school.Methods:The diabetes self-management school was founded.It took many forms of diabetes management and training.Results:The management number of patients with diabetes mellitus,diabetes management rate,standardized management rate and blood glucose control rate were significantly improved.Conclusion:Through the establishment of diabetes self-management school in the community health service center,strengthening the chronic disease management knowledge training of general practitioner team,training and selecting community volunteers from patients,providing peer support services for the patients with diabetes of whole area can make the diabetes community management achieve greater successes.
