护理干预在社区老年慢性病患者健康管理中的作用

The effect of nursing intervention on the health management of elderly patients with chronic diseases in community

目的:分析护理干预在社区老年慢性病患者健康管理中的应用价值。方法:选取老年慢性病患者80例,随机分为两组,对照组采用常规健康管理,观察组在对照组基础上加护理干预,比较两组患者身体变化情况。结果:通过2年的护理干预,观察组在健康知识掌握情况、血压变化、糖尿病空腹血糖以及日常生活能力评分方面均优于对照组,差异具有统计学意义(P<0.05)。结论:给予社区老年慢性病患者适当的护理干预可以提高患者对健康知识的掌握能力,改善疾病状况。

Objective:To investigate the value of nursing intervention on the health management of elderly patients withchronic diseases in community.Methods:80 elderly patients with chronic diseases were selected.They were randomly dividedinto two groups.Patients in the control group used conventional health management.Patients in the observation group combinedwith nursing intervention on the basis of the control group.We compared the physical changes of patients in two groups.Results:After 2 years of nursing intervention,health knowledge,change of blood pressure,diabetes fasting blood glucose anddaily life ability score of the observation group was better than those of the control group,and the differences were statisticallysignificant(P < 0.05).Conclusion:Appropriate nursing intervention for elderly patients with chronic diseases in the communitycan improve the patient’s ability to master the health knowledge and improve the disease status.
