斑点追踪技术测量二尖瓣环位移在评价心力衰竭患者左心功能中的临床研究

摘要 目的:探讨斑点追踪技术测量二尖瓣环位移(MAD)对心力衰竭患者的左心功能的评价。方法:收治心力衰竭患者65例,根据射血分数分为两组,将31例射血分数正常患者归为A组,34例射血分数≤40%患者归为B组,并选取30例健康志愿者作为对照组。采用斑点追踪技术(STI)检测3组MAD参数。结果:A、B两组MAD各参数均明显低于对照组,B组MAD各参数亦显著低于A组(P<0.05)。与对照组相比,A、B两组的LVEDV和LVESV均显著增高,LVEF显著降低,B组的LVEF显著低于A组,差异具有统计学意义(P<0.05)。MAD参数与LVEF呈正相关。结论:斑点追踪技术通过测量MAD可为评价心力衰竭患者左心功能提供重要参考。 Objective:To investigate the evaluation of mitral annular displacement(MAD) measured by speckle tracking imagingon the left ventricular function of patients with heart failure.Methods:According to the ejection fraction,65 cases of patients withheart failure were divided into two groups.31 cases with normal ejection fraction were grouped into A;34 cases with ejectionfraction less than 40% were grouped into B.30 cases of healthy volunteers were selected as the control group.The MAD of thethree groups were detected with speckle tracking imaging(STI).Results:The MAD parameters of group A and group B weresignificantly lower than those in the control group,and the MAD parameters of group B were also significantly lower than that ofgroup A (P<0.05).Compared with the control group,the LVEDV and LVESV of group A and group B were significantly higher;LVEF were significantly lower;the LVEF of group B was significantly lower than that of group A;the differences were statisticallysignificant(P<0.05).The MAD parameters were positively correlated with LVEF.Conclusion:Mitral annular displacementmeasured by speckle tracking imaging could provide an important reference for the evaluation of left ventricular function ofpatients with heart failure.
