高龄股骨粗隆间骨折患者人工股骨头置换治疗体会

Experience of artificial femoral head replacement in the treatment of elderly patients with intertrochanteric fracture

目的:分析高龄股骨粗隆间骨折患者应用人工股骨头置换的优点和手术技巧。方法:收治高龄股骨粗隆间骨折患者32例,给予人工股骨头置换治疗,分析手术方式和疗效。结果:32例患者术后骨折重建良好,固定稳定,人工股骨头植入满意。术后无死亡病例,无并发症发生。22例恢复至受伤前生活状态,10例髋关节活动稍差,但基本生活能够自理。髋关节Harris评分平均84分,效果良好。结论:对于高龄股骨粗隆间骨折患者,人工股骨头置换治疗可以早期负重活动,减少长期卧床的并发症。术中采用先重建后植入的方法,方法简便、手术精度高,基本上达到了股骨颈骨折人工股骨头置换的疗效,效果满意。

Objective:To analyze the advantages and surgical techniques of artificial femoral head replacement in the elderlypatients with intertrochanteric fracture.Methods:32 elderly patients with intertrochanteric fracture were selected.They were givenartificial femoral head replacement treatment.The surgical methods and curative effects were analyzed.Results:The postoperativefracture reconstruction of 32 patients was good;it was fixed and stable;the artificial femoral head implant was satisfactory.Therewas no death after operation,and no complication.22 cases recovered to the life state before injury.10 cases of hip activity wereslightly worse,but the basic life could take care of themselves.The hip joint Harris score average was 84 scores,and the effect wasgood.Conclusion:For elderly patients with intertrochanteric fracture,the artificial femoral head replacement treatment can be anearly weight bearing activity,reduce the complications of long-term bed ridden.The first reconstruction after implantation methodis used during operation;the method is simple;the operation precision is high.It basically achieves the curative effect of artificialfemoral head replacement for femoral neck fracture,and the effect is satisfactory.
