中度支气管肺发育不良早产儿潮气呼吸肺功能的研究

Study on the tidal breathing lung function of preterm infants with moderate bronchopulmonary dysplasia

目的:观察中度支气管肺发育不良早产儿潮气呼吸肺功能各项指标的变化。方法:收治中度支气管肺发育不良早产儿(观察组)26例,非支气管肺发育不良早产儿(对照组)28例;采用潮气呼吸法测定两组早产儿的潮气呼吸肺功能的各项指标。主要参数:每分通气量(MV),呼吸频率(RR),潮气量(VT/kg),吸呼比(TI/TE),达峰时间(TPEF),达峰时间比(TPEF/TE),达峰容积(VPEF),达峰容积比(VPEF/VE)。结果:与对照组比较,观察组TI/TE、TPEF/TE、VPEF/VE降低,Vt/kg下降,差异均有统计学意义(P<0.05)。结论:中度支气管肺发育不良早产儿存在小气道阻塞及肺顺应性的下降,潮气呼吸肺功能可作为新生儿呼吸监护重要的无创手段。

Objective:To observe the changes of adverse tidal breathing lung function indexes of preterm infants with moderatebronchopulmonary dysplasia.Methods:26 premature infants with moderate bronchopulmonary dysplasia(the observation group)were selected,28 cases of premature infants(the control group)with non-bronchial pulmonary dysplasia were also selected.Theindexes of tidal breathing lung function were measured in the two groups of premature infants by tidal breathing method.The mainparameters:minute ventilation(MV),respiratory rate(RR),tidal volume(VT/kg),the respiratory ratio(Ti/TE),time to peak(TPEF),timeto peak ratio(TPEF/TE)and peak volume(VPEF)of peak volume ratio(VPEF /VE).Results:Compared with the control group,the Ti/Te,TPEF/TE,VPEF/VE and Vt/kg of the observation group decreased,with significant differences(P<0.05).Conclusion:Thedecrease of lung compliance and airway obstruction exist in preterm infants with moderate bronchial and pulmonary dysplasia,tidal breathing lung function can be used as an important non-invasive means of neonatal respiratory monitoring.
