D-二聚体升高的结直肠肿瘤术前预防性应用低分子肝素的探讨

Investigation of low molecular weight heparin applied preventively before surgery in the treatment of colorectal neoplasms with high D-dimer

目的:探讨术前预防性使用低分子肝素对D-二聚体升高的结直肠肿瘤患者的影响。方法:收治结直肠肿瘤患者36例,分为3组。干预组给予低分子肝素钙治疗,对照组予以等量生理盐水,正常组不给予抗凝相关处理。比较3组血浆D-二聚体水平及白细胞、中性粒细胞、C反应蛋白、降钙素原水平。结果:干预组术后第7天D-二聚体水平明显低于对照组,差异具有统计学意义(P<0.05)。干预组术后第4天CRP、PCT水平低于对照组,差异具有统计学意义(P<0.05)。结论:术前预防性应用低分子肝素,能一定程度上降低患者术后D-二聚体水平,减轻炎性反应。

Objective:To investigate the effect of low molecular weight heparin applied preventively before surgery in patients with colorectal neoplasms combined with high D-dimer.Methods:36 cases of patients with colorectal neoplasms were divided into three groups.The intervention group was treated with low molecular weight heparin calcium.The control group was given amount of normal saline.The normal group was not given anticoagulation correlation processing.The plasma levels of D-dimer and the levels of white blood cells,neutrophils,C reactive protein and calcitonin in the three groups were compared.Results:The level of D-dimer in the intervention group on the seventh day after operation was significantly lower than that in the control group,and the difference was statistically significant (P<0.05).The levels of CRP and PCT in the intervention group was lower than that in the control group on the fourth day after operation,and the difference was statistically significant (P<0.05).Conclusion:Preoperative prophylactic application of low molecular weight heparin could reduce the level of D-dimer to a certain extent and reduce the inflammatory reaction.
