超声内镜在胃间质瘤诊断与治疗中的应用价值

Application value of endoscopic ultrasonography in the diagnosis and treatment of gastric stromal tumors

目的：探讨超声内镜在胃间质瘤诊断与治疗中的应用价值。方法：收治经超声内镜诊断的胃间质瘤患者23例，并对镜下手术治疗效果进行总结。结果：超声内镜可明确显示瘤体位置及形态，准确进行肿瘤侵袭危险性分级。23 例中瘤体最大直径＜3 cm 5 例，行超声内镜下手术治疗，18 例经外科手术治疗。出院随访3～12 个月，均无肿瘤复发与远处转移。结论：超声内镜诊断胃间质瘤优势明显，能准确进行肿瘤侵袭危险性分级，指导治疗方案，对合适病例行超声内镜下手术安全、疗效确切，临床应严格选择镜下手术适应证。

Objective:To investigate the application value of endoscopic ultrasonography in the diagnosis and treatment of gastricstromal tumors.Methods:23 patietns with gastric stromal tumors diagnosed by endoscopic ultrasonography were selected,and wesummarized the effect of surgical treatment under the microscope.Results:Ultrasound can clearly show the location and shape ofthe tumor,and accurate classification of the risk of tumor invasion.In 23 cases,there were 5 cases with the largest diameter lessthan 3 cm,and the patients were treated by endoscopic surgery.18 cases were treated by surgical operation.The patients werefollow-up for 3 to 12 months after discharged from the hospital,there was no tumor recurrence and distant metastasis.Conclusion:Endoscopic ultrasonography in the diagnosis of gastric stromal tumors has obvious advantages.It can accurately carry out the riskclassification of tumor invasion and guide treatment program.It is safe and effective for a suitable cases underwent endoscopicultrasound surgery.The indications of surgical indications should be strictly selected in clinic.
