兔耳创伤后创面即刻外用药物预防瘢痕增生的实验研究

摘要 目的:本实验观察兔耳创伤后创面立即外用曲安奈德与维拉帕米预防瘢痕增生的疗效。方法:健康的新西兰大耳白兔48只,制作兔耳创伤模型,随机分为A,B,C三组,每组16只计创面32个,A组为外用曲安奈德霜,B组外用维拉帕米霜,C组为生理盐水对照组。术后第一天起用药至4周后兔耳上皮化,切取瘢痕组织计算瘢痕增生指数,观察masson染色胶原纤维的形态分布及成纤维细胞数量。结果:A组与B组比较差异有统计学意义(P<0.05),A组、B组与C组比较,差异皆具有统计学意义,差异显著(P<0.01)。结论:兔耳创伤后创面立即外用曲安奈德与维拉帕米能有效预防抑制瘢痕增生,曲安奈德作用强于维拉帕米,但曲安奈德可能使皮肤萎缩,甚至坏死。 Objective The experiment observes the effect of immediate external application of triamcinolone acetonide andverapamil on the prevention of scar hyperplasia in the early stage of traumatic wound of rabbit ears. Methods 48 healthyNew Zealand white rabbits were randomly divided into A and B and C three groups, 16 in each group, 32 traumatic woundsaltogether. Group A was used triamcinolone acetonide,group B was used verapamil as the experimental group, and Group C wasused normal saline as control group. From first day after surgery to 4 weeks , traumatic wounds of rabbit ear were epithelial,the scar tissue was taken to calculate the hypertrophic index and observe Masson staining of collagen fiber morphologicaldistribution and the number of fibroblasts. Results There was difference between group A and group B(P < 0.05). There wassignificant difference between group A,B and group C(P < 0.01). Conclusion It is concluded that triamcinolone acetonideand verapamil can effectively prevent the formation of hypertrophic scar in rabbit ears. Triamcinolone acetonide has a bettereffect of inhibition than verapamil on the formation of hypertrophic scar. But triamcinolone acetonide may cause skin atrophy oreven necrosis.
