静态姿势图在良性阵发性位置性眩晕中感觉结构变化研究 被引量:6

摘要 目的通过静态姿势图可分析BPPV患者感觉结构(Sensory structure)变化的特征;明确BPPV患者康复训练后感觉结构变化情况;方法选取符合诊断标准的BPPV患者,采用静态姿势仪进行感觉结构变化评估前庭康复,定量评价眩晕患者在消除或减弱视觉和本体觉的情况下前庭功能及半规管功能,并采用姿势图分析前庭功能低下时视觉前庭,脑干小脑,和脊髓本体觉的能量分布变化;结果静态姿势图可评估BPPV患者发病时在消除或减弱视觉系统和本体觉系统功能时,明确前庭系统中半规管功能低下的情况。;BPPV患者发病时要维持平衡,则需要通过视觉或本体觉加强,从而发现眩晕患者维持平衡很多程度依靠视觉和小脑脑干平衡功能;运用前庭康复系统按要求对外周性眩晕患者进行平衡康复训练,而且前庭功能测试后前庭评分恢复正常且显著高于对照组(P<0.05);结论 BPPV患者大部分出现前庭系统功能评分低,并能发现前庭半规管功能低下时评分低的半规管侧别;评价BPPV患者在不同状态下患者维持平衡很多程度依靠视觉和小脑平衡功能;前庭康复对BPPV患者疗效明确。 Objective Their characteristics of static posturography for peripheral vertigo function parameters in patientswith BPPV were analyzed,Static posturography can clearly assess balance function of peripheral vertigo patients andSensory structure after rehabilitation Methods Patients with peripheral vertigo with BPPV weresensory organization testand vestibular rehabilitation . With clinical treatment of peripheral vertigo of BPPV they were divided into a control groupand vestibular disorders , The energy distribution of visual vestibular, brainstem, and spinal cord Results Static posture diagramevaluation BPPV patients show when eliminated or weaken the vision system and the proprioceptive system function,the function of semicircular canal of vestibular system is low mainly; BPPV patients to maintain a balance need rely on visualor proprioceptive sense, and find balance in patients with vertigo largely rely on visual and cerebellum brainstem balancefunction; balance rehabilitation training according to the requirements of peripheral vertigo patients with vestibular rehabilitationsystem and vestibular function scores returned to normal and significantly higher than that of the control group (P <0.05) Conclusion BPPV patients most of the vestibular system function score low, and can be detected low vestibular semicircularcanal function ,scores lower semicircular canal side and the evaluation of patients to maintain the balance largely onvisual and cerebellum balance function; vestibular rehabilitation on the patients with BPPV clearly effective.
