探讨胸腔镜食管癌根治术联合胸内吻合术治疗食管癌的临床效果

Exploration on the clinical effect of thoracoscopic esophageal resection combined with intrathoracic anastomosis in the treatment of esophageal cancer

目的:探讨胸腔镜食管癌根治术联合胸内吻合术治疗食管癌的临床效果。方法:收治食管癌患者45例,随机分为两组。对照组采用传统开胸食管癌根治术,试验组采用胸腔镜食管癌根治术联合胸内吻合术。比较两组患者手术一般情况和术后并发症及手术前后外周静脉血各炎性因子表达水平。结果:试验组术中出血量、术后胸腔引流量、住院时间和术后并发症发生率均低于对照组(P<0.01);与术前比较,术后两组各项炎性因子水平均有所升高(P<0.01),且对照组显著高于试验组(P<0.05或P<0.01)。结论:胸腔镜食管癌根治术联合胸内吻合术治疗食管癌创伤小、可降低术后并发症发生率和减少术后炎症反应。

Objective:To explore the clinical effect of thoracoscopic esophageal resection combined with intrathoracic anastomosis in the treatment of esophageal cancer.Methods:45 patients with esophageal cancer were selected,they were randomly divided into the two groups.The control group was treated with traditional open chest esophageal cancer radical surgery,the experimental group was treated with thoracoscopic esophageal resection combined with intrathoracic anastomosis,the general conditions of operation and postoperative complications,and the expression of inflammatory factors in peripheral venous blood before and after operation were compared between the two groups.Results:In the experimental group,the amount of bleeding,postoperative thoracic drainage,hospital stay and postoperative complications were lower than those in the control group(P<0.01).Compared with preoperative,the levels of inflammatory factors in the two groups were increased(P<0.01),and the control group was significantly higher than the experimental group(P<0.05 or P<0.01).Conclusion:Thoracoscopic esophageal resection combined with intrathoracic anastomosis in the treatment of esophageal cancer with less invasive and can reduce the incidence of postoperative complications and postoperative inflammatory reaction.
