循证护理在急性心肌梗死并发心律失常患者中的应用

The use of evidence-based nursing in patients with acute myocardial infarction and arrhythmia

目的探讨循证护理在急性心肌梗死并发心律失常患者中的应用效果，寻找有效的护理途径。方法连续入选2013年7月至2014年6月在安阳市人民医院心内科住院的急性心肌梗死并发心律失常的患者98例，其中男性47例，女性51例，年龄46~83岁，平均（60.8±8.3）岁。随机分为对照组和观察组，各49例。对照组采用常规护理模式，观察组在对照组的基础上予以循证护理。比较两组出院时住院时间、卧床时间、心律失常发生情况，美国纽约心脏协会（NYHA）心功能分级以及满意度。结果观察组卧床时间、住院时间以及心律失常发生率较对照组减少，差异有统计学意义（P均＜0.05）。观察组的心功能情况明显优于对照组，差异有统计学意义（P均＜0.05）。观察组总满意率明显高于对照组（91.84%vs.71.43%），差异有统计学意义（P＜0.05）。结论循证护理较常规护理可以缩短急性心肌梗死并发心律失常患者治疗时间，降低心律失常的发生率，提高临床治疗满意度。

Objective To explore the application effect of evidence-based nursing in patients with acute myocardial infarction complicated with arrhythmia, and to look for the better ways of nursing. Methods From July 2013 to October 2014, 98 patients (including 47 males, 51 females, age= 46 -83, average age= 60.8±8.3) with acute myocardial infarction combined with arrhythmia were randomly divided into observation group and control group, Patients in the control group were given by conventional care. On the basis of routine nursing, the experimental group was carried out by the evidence-based nursing. The classification of cardiac arrhythmia,cardiac function classification, average length of stay and bed time were compared. Results Malignant arrhythmia incidence, bed rest time and average length of hospital stay of the observation group were lower than that of the control group (P<0.05). Cardiac function of the observation group was better than that of the control group (P<0.05).Patient satisfaction of the observation group was higher than that of the control group (91.84% vs. 71.43%, P<0.05).Conclusion Evidence-based nursing can effectively reduce the incidence of arrhythmia and treatment time, and increase patient satisfaction.
