摘要
目的:通过新生儿肺炎的血乳酸监测,指导新生儿肺炎病情判断、预后评估。方法:对237例新生儿肺炎患儿进行动脉血气测定,以及血常规、肝功能、肾功能、胸部X线、心肌酶检测等检查,根据血乳酸水平将其分为正常组及异常组。对比两组的动脉氧分压(PaO_2)、并发症发生的情况。结果:血乳酸水平>2.2 mg/L 72例(30.4%)为异常组。异常组的动脉氧分压(PaO_2)低于正常组(P<0.05);异常组的并发症发生例数高于正常组(P<0.05)。结论:监测患儿的血乳酸的浓度,可了解新生儿缺氧的程度,指导呼吸,对症治疗,从而改善患儿缺氧状态,减少并发症。
Objective:Through monitoring of blood lactic acid in neonatal pneumonia,to guide the condition judgment andprognosis evaluation of neonatal pneumonia.Methods:237 cases of neonatal pneumonia were given arterial blood gas determination,and blood routine,liver function,renal function,chest X-ray,myocardial enzyme detection.According to the blood lactic acid level,they were divided into the normal group and the abnormal group.The arterial oxygen partial pressure(PaO2) and the incidence ofcomplications were compared between the two groups.Results:72 cases(30.4%) of blood lactic acid level >2.2 mg/L were as theabnormal group.The arterial oxygen partial pressure(PaO2) of the abnormal group was lower than that of the normal group(P<0.05).The incidence cases of complications in the abnormal group was higher than that of the normal group(P<0.05).Conclusion:Monitoring the concentration of blood lactic acid in children can understand the degree of neonatal hypoxia,guide the symptomatictreatment of respiratory disease,so as to improve the anaerobic condition of children,reduce the complications.
