肝胆外科手术后胆漏的临床治疗体会

Clinical experience on the treatment of bile leakage after operation in department of hepatobiliary surgery

目的:探讨肝胆外科术后胆漏症状治疗方式及临床疗效。方法:收治肝胆外科手术患者56例,所有患者均出现胆漏症状,分为对照组和观察组各28例,对照组采用传统开腹胆囊切除术,观察组采用腹腔镜胆囊切除术。结果:对照组3周治愈9例(32.1%),5周治愈19例(67.9%);观察组3周治愈18例(64.3%),5周治愈26例(92.9%);两组胆漏治疗效果差异明显,观察组疗效明显优于对照组(P<0.05)。结论:胆漏是肝胆外科常见并发症之一,若不及时处理会引起盆腔炎、肠间炎等诸多症状,腹腔镜手术治疗效果优于传统开腹手术。

Objective:To investigate the treatment methods and clinical efficacy of biliary leakage after operation in thedepartment of hepatobiliary surgery.Methods:56 patients in the department of hepatobiliary surgery were selected.All of thosepatients had symptoms of bile leakage.They were divided into the control group and the observation group with 28 cases in each.Patients in the control group were treated with traditional open cholecystectomy,while in the observation group were treated withlaparoscopic cholecystectomy.Results:9 cases(32.1%) in the control group were cured at 3 weeks,and 19 cases(67.9%) were curedat 5 weeks;18 cases(64.3%) in the observation group were cured at 3 weeks and,and 26 cases(92.9%) were cured at 5 weeks;thetreatment effect of bile leakage was significantly different between groups,and the observation group was better than the controlgroup(P<0.05).Conclusion:Bile leakage is one of the common complications in the department of hepatobiliary surgery.It cancause pelvic inflammatory disease,intestinal inflammation and many other symptoms without timely treatment.Laparoscopicsurgery has better treatment effect on Bile leakage than the traditional open surgery.
