维持性血液透析患者中心静脉导管血栓及纤维鞘形成的护理研究

Nursing care of patients with central venous catheter thrombosis and fibrous sheath formation in maintenance hemodialysis

目的探讨维持性血液透析患者中心静脉导管血栓与纤维鞘形成的护理干预措施及效果。方法选取本院2014年1月至2016年1月采用中心静脉导管行维持性血液透析患者136例,随机分为对照组和观察组各68例,对照组采用常规临床护理干预,观察组给予循证护理模式。观察两组患者血栓与纤维鞘形成率,比较患者护理后的凝血功能。结果两组患者护理后凝血功能指标比较差异无统计学意义(P>0.05),但观察组患者中血栓与纤维鞘形成的一次发生率与再次发生率均明显低于对照组,差异具有统计学意义(P<0.05)。结论维持性血液透析患者采用循证护理模式可减少中心静脉导管血栓及纤维鞘形成的发生率,避免出现导管功能不良事件,提高临床治疗效果。

Objective To explore the effect and nursing measures of patients with central venous catheter thrombosis and fibrous sheath formation in maintenance hemodialysis.Methods One hundred and thirty-six patients of maintenance hemodialysis with central venous catheter were selected in our hospital from January 2014 to January 2016,and were randomly divided into the control group and the observation group(n=68).The patients in control group were treated with routine nursing,and the patients in observation group were given evidence-based nursing.The formation rate of thrombosis and fibrous sheath were observed in the two groups,and the coagulation function of patients after nursing was compared.Results There was no significant difference in coagulation function indexes between the two groups after nursing(P>0.05).But the incidence of thrombosis and the formation of fibrous sheath of observation group were significantly lower than that of the control group,with statistically significant(P<0.05).Conclusion Evidence-based nursing can reduce the incidence of thrombosis and fibrous sheath of central venous catheter,avoid catheter function adverse events,improve the clinical treatment effect.
