氯氮平辅助利培酮治疗难治性精神分裂症患者的效果观察

Observation ofeffects of Risperidonecombined with adjuvant Clozapinein treatment of refractory schizophrenia

目的:探究氯氮平辅助利培酮治疗难治性精神分裂症患者的疗效及安全性。方法:选取难治性精神分裂症患者80例作为研究对象,随机分为对照组和观察组,每组各40例。对照组患者给予利培酮片治疗;观察组患者在对照组的基础上加服氯氮平片。比较两组患者的治疗效果、PANSS总分变化及不良反应情况。结果:观察组患者的总有效率为95.00%,明显高于对照组的77.50%,两组患者差异有统计学意义(P<0.05)。治疗后2周、4周、8周,两组患者的PANSS总分持续下降,但观察组患者的下降速度更快(P<0.05);且两组患者的PANSS总分于治疗后8周与治疗前比较,差异有统计学意义(P<0.01)。观察组患者的不良反应发生率为22.50%,对照组为20.00%,差异无统计学意义(P>0.775)。结论:氯氮平辅助利培酮治疗难治性精神分裂症患者的疗效较优,且安全性较好。

Objective: To study effects and safety of Clozapine and risperidone in treatment of refractory schizophrenia. Methods:80 patients with refractor^^ schizophrenia were selected asthe research objects and randomly divided into control group ( n = 4 0 ) and observation group. The control group was treated with Risperidone, while the obser^^ation group was treated with Clozapine combined with Risperidone. The treatment effects, PANSS total score change and adverse reactions were compared between the two groups.Results: The total effective rate of observation group was higher than that of control group[95. 00% vs. 77. 5 0 % ] , and there was a significant difference between the two groups ( P < 0 . 0 5 ) . 2 , 4 and 8 weeks after the treatment, the PANSS total scores of the two groups continued to decline, but the decline rate of observation group was faster ( P < 0 . 0 5 ) . The differences of the PANSS total score before and 8 weeks after the treatment of the two groups were statistically significant( P < 0 . 0 1 ) . The adverse reaction incidence of observation group was 2 2 .5 0 % , while that of control group was 2 0 .0 0 % , and the difference was not significant ( P > 0 . 0 5 ) . Conclusions: Risperidone combined with adjuvant Clozapineis more effective in the treatment of refractory schizophrenia and has a better safety.
