彩色多普勒超声显像检查在诊断新生儿肺透明膜病中的临床应用 被引量:15

摘要 目的对新生儿肺透明膜病的X线平片和肺部超声诊断的结果进行对比分析。方法对临床怀疑为肺透明膜病(Hyaline Menbrane Diseases,HMD)的78例早产儿的肺部进行X线和肺部超声检查,对检查结果进行分析,并对并发症的检出率进行统计学分析。结果 X线检查示所有78例患儿中,HMD 1级有15例,HMD 2级有39例,HMD 3级有18例,HMD 4级有6例。肺部超声检查示总共48例HMD患儿在超声下见肺实变及支气管充气征。56例患儿均有A线消失、胸膜线异常及肺水肿或肺泡-间质综合征。24例患儿有肺滑消失与肺搏动,3例2级HMD患儿未见肺搏动。5例患儿有不同程度的胸腔积液。并发症的检出率可见超声组的检出率明显高于X线组,结果具有统计学意义(P<0.05)。结论肺部超声检查可做为诊断HMD的一种新方法,它具有实用、简便、无创伤并可重复检查的优点,且对于并发症的检出率超声检查优于X线。 Objective To compare the plain X-ray radiography and lung ultrasonography examinationsin diagnosis of the newborn hyaline membrane disease (HMD). Methods The X-ray and ultrasonicdiagnosis of the lung of the 78 neonates that were suspected to the HMD were carried and then the resultswere analyzed. Meanwhile, the detection rate of the complications was analyzed by using the statistics.Results The results of the X ray showed that 15 HMD was the first level, 39 HMD was the second level,18 HMD was the third level and 6 HMD was the fourth level. The results of the lung ultrasonic diagnosisshowed that the lung consolidation and air bronchogram could be seen in the lung ultrasonography. The Aline abolition, pleural abnormalities, pneumonedema and alveolus-mesenchymal syndromes were foundin the 56 HMD. The pulmonary smooth disappearance and lung beat were found in the 24 HMD. Thelung beat were not found in the 3 Second level HMD. The different level pleural effusion was found inthe 5 HMD. The complications detection rate of the hyperacoustic group was higher than the X-ray groupand the results were statistical significance (P<0.05). Conclusion The lung ultrasonography is a new wayfor the diagnosis of the HMD. It is featured with the advantages of usefulness, convenience, non-invasionand repeatability in examinations. The detection rate of the complications with the ultrasonographysurpasses the X-ray.
