中晚期宫颈癌外照射阶段调强放疗与三维适形放疗的剂量对比研究

Dose comparison study of intensity modulated radiation therapy and three-dimensional conformal radiotherapy during external irradiation stage in the treatment of middle and advanced cervical cancer

目的:探讨中晚期宫颈癌外照射阶段调强放疗与三维适形放疗的剂量对比研究。方法:收治中晚期宫颈癌患者50例。所有患者中采用三维适形放疗25例,采用调强放疗25例。比较两种放疗方法的临床疗效。结果:在调强放疗计划中CI平均0.98,高于三维适形放疗(0.58),更接近于1;调强放疗计划中HI为0.87,低于三维适形计划中的1.12;直肠、膀胱、肠道、股骨头在调强放疗计划中受照射的体积均明显小于三维适形放疗计划;肠道的最大剂量在调强放疗计划中低于三维适形放疗计划,差异均有统计学意义(P<0.05)。结论:调强放疗计划治疗中晚期宫颈癌可获得更高的治疗增益比。

Objective:To investigate the dose comparison of intensity modulated radiation therapy and three-dimensional conformalradiotherapy during external irradiation stage in the treatment of middle and advanced cervical cancer.Methods:50patientswith middle and advanced cervical cancer were selected,three dimensional conformal radiotherapy was used in25patients,and25cases were modulated by intensity modulated radiation therapy,we compared the effectiveness of the two methods of radiotherapy.Results:In the intensity modulated radiation therapy,the mean of CI was0.98,higher than three dimensional conformal radiationtherapy of0.58,closer to1,HI was0.87in the intensity modulated radiation therapy plan,lower than1.12in the three dimensionalconformal plan.The volume of the rectum,bladder,intestine and head of the femoral head in the intensity modulated radiation therapyplan were significantly smaller than the three dimensional conformal radiotherapy plan.The maximum dose of the intestinaltract in the intensity modulated radiation therapy is lower than the three dimensional conformal radiotherapy plan,the differenceswere statistically significant(P<0.05).Conclusion:Radiotherapy for the treatment of advanced cervical cancer can get a highertherapeutic benefit ratio.
