原发性闭角型青光眼伴白内障给予同期治疗的术式研究

Operation study on the concurrent treatment of primary angle closure glaucoma with cataract

目的:研究原发性闭角型青光眼伴白内障给予同期治疗的最佳术式。方法:研究组首先给予前房角镜测定房角关闭范围,若>180°给予白内障超声乳化吸除人工晶体植入联合房角分离术,若<180。则给予白内障乳化吸除人工晶体植入术。对照组给予青光眼白内障联合手术。对比两组治疗前后的眼压、视力、眼房角开放度。对比两组并发症。结果:研究组治疗后眼压、视力及眼房角开放度数均显著优于对照组(P<0.05)。研究组并发症发生率为0%,显著低于对照组的16.98%。结论:与青光眼白内障联合手术相比,白内障超声乳化吸除(或联合房角分离术)治疗原发性闭角型青光眼伴白内障具有更佳的疗效,且并发症少。

Objective:To study the optimal operation of the concurrent treatment of primary angle closure glaucoma with cataract.Methods:The study group was given gonioscope to determine the corner closure range.Patients with more than180degrees wasgiven cataract ultrasonic emulsification gettering artificial lens implantation combined with corner separation operation,andpatients with less than180degrees was given cataract ultrasonic emulsification gettering artificial lens implantation.The controlgroup was given glaucoma and cataract combined surgery.The intraocular pressure,visual acuity,eye corner open degree beforeand after the treatment were compared between groups.The complications of two groups were compared.Results:After treatment,the intraocular pressure,visual acuity,eye corner open degree of the study group were significantly better than those of the controlgroup(P<0.05).The incidence rate of complications in the study group was0,which was significantly lower than16.98%of thecontrol group.Conclusion:Combined with glaucoma and cataract combined surgery,the phacoemulsification(or combined withcorner separation operation)in the treatment of primary angle closure glaucoma with cataract has a better curative effect and lesscomplication.
