急诊胸腹部创伤中螺旋CT的临床应用价值分析

Clinical application value of spiral CT in emergency thoracic and abdominal trauma

目的:研究、分析螺旋CT在急诊胸腹部创伤中的应用价值。方法:收治胸腹部创伤患者63例,对其急诊X线、B超、螺旋CT的检查结果进行统计分析。结果:34例胸部损伤中肋骨骨折31例,胸片未检出2例,血胸7例,胸片未检出2例,气胸20例,纵隔气肿4例,锁骨骨折3例;32例腹部损伤者中脾脏损伤20例,B超未检出2例,肾脏损伤16例,肝脏损伤12例,肠系膜损伤8例,B超没有特异性诊断表现。结论:螺旋CT相较于X线与B超检查,对于胸腹部创伤能够给予更高的诊断率、更准确的损伤定位和病情评估,临床上需要根据实际情况予以检查方式的选择。

Objective:To study the application value of spiral CT in emergency thoracic and abdominal trauma.Methods:63patients with emergency thoracic and abdominal trauma were selected.The results of X-ray,B ultrasound and spiral CT wereanalyzed statistically.Results:There were31cases of rib fractures in34cases of thoracic injuries;chest radiographs were notdetected in2cases;hemothorax in7cases;chest radiographs were not detected in2cases;20cases of pneumothorax;4cases ofmediastinal emphysema;3cases of clavicular fractures.There were20cases of splenic injury in32cases of abdominal trauma;2cases were not detected by B ultrasound;renal injury in16cases;12cases of liver injury;8cases of mesenteric injury;there was nospecific diagnostic performance by B-mode ultrasonography.Conclusion:Compared with X-ray and B ultrasound examination,spiral CT can give a higher diagnosis rate,more accurate damage location and condition assessment for thoracic and abdominaltrauma.In clinical,it need to be checked according to the actual situation of choice.
