摘要
目的探讨多维度系统化护理对妊娠期肝内胆汁瘀积症(ICP)患者妊娠结局和认知度的影响。方法 ICP患者82例随机分为观察组和对照组,各41例。对照组采用常规护理,观察组在对照组的基础上给予多维度系统化护理干预。对比2组护理前后疾病认知度,观察2组护理后血清总胆红素、血清胆汁酸和丙氨酸氨基转移酶水平及妊娠结局。结果观察组护理后疾病认知程度评分高于对照组,差异有统计学意义(P<0.01)。观察组护理后血清总胆红素、血清胆汁酸和丙氨酸氨基转移酶水平低于对照组,差异有统计学意义(P<0.05)。观察组胎儿宫内窘迫、早产、羊水污染、剖宫产、产后出血发生率均低于对照组,差异有统计学意义(P<0.05)。结论多维度系统化护理运用于ICP患者可明显提高患者对疾病的认知,减轻临床症状,改善妊娠结局。
Objective To investigate the effect of multidimensional systematic care on pregnancy outcome and disease awareness of patients with intrahepatic cholestasis of pregnancy( ICP).Methods A total of 82 ICP patients were randomly divided into two groups,with 41 cases in each group. Both two groups were given routing nursing care,and the observation group was applied with multidimensional systematic care. The disease awareness of patients was assessed before and after intervention. Clinical parameters including serum total bilirubin,serum bile acid and alanine aminotransferase,as well as pregnancy outcomes were compared between two groups. Results Patients in the observation group had higher disease awareness compared with those in the control group after nursing intervention( P < 0. 01). Levels of serum total bilirubin,serum bile acid and alanine aminotransferase were lower in the observation group than those in the control group after nursing intervention( P< 0. 05). The incidence of fetal distress,premature delivery,amniotic fluid contamination,cesarean section and postpartum hemorrhage were lower in the observation group than those in the control group,with significant difference( P < 0. 05). Conclusions Multidimensional systematic care used in ICP patients can significantly increase disease awareness and improve pregnancy outcome.
