妊娠期肝内胆汁淤积症120例的治疗与妊娠结局研究

Research of treatment and pregnancy outcome of 120 cases with intrahepatic cholestasis of pregnancy

目的:探讨妊娠期肝内胆汁淤积症(ICP)对孕妇的妊娠结局影响。方法:收治ICP患者120例作为试验组,给予熊去氧胆酸片、地塞米松片与苯巴比妥片治疗。选择同期健康孕妇100例作为对照组。比较两组血清丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、总胆汁酸(TBA)、直接胆红素(DBIL)及不良妊娠结局。结果:试验组治疗前ALT、TBIL、TBA、DBIL显著高于对照组(P<0.05)。治疗后,试验组ALT、TBIL、TBA、DBIL显著低于治疗前(P<0.05)。试验组产后出血率、早产与剖宫产率显著高于对照组(P<0.05)。试验组不良结局发生率显著高于对照组(P<0.05)。结论:ICP对孕妇与新生儿的影响较大,早发现、早治疗可降低ICP对母婴的危害。

Objective:To investigate the effect of intrahepatic cholestasis of pregnancy (ICP) on the pregnancy outcome of pregnant women.Methods:120 cases of patients with ICP were as the experiment group and treated with ursodeoxycholic acid tablets,dexamethasone tablets and phenobarbital tablets.100 cases of healthy pregnant women were as the control group.The serumalanine aminotransferase (ALT),total bilirubin (TBIL),total bile acid (TBA),direct bilirubin (DBIL) and adverse pregnancyoutcomes between groups were compared.Results:The ALT,TBIL,TBA and DBIL in the experiment group before treatment weresignificantly higher than those in the control group (P<0.05).After treatment,the ALT,TBIL,TBA,DBIL of the experiment groupwere significantly lower than before treatment (P<0.05).The postpartum hemorrhage rate,premature delivery rate and cesarean section rate in the experiment group were significantly higher than those in the control group (P<0.05).The incidence of adverse outcomes in the experiment group was significantly higher than that in the control group (P<0.05).Conclusion:ICP had great influence on pregnant women and newborns.Early detection and early treatment could reduce the harm of ICP to mothers and infants.
