益肾化瘀方治疗卵巢储备功能降低临床研究

Clinical Study on Treatment of Decreased Ovarian Reserve by Yishen Huayu Decoction

目的观察益肾化瘀方治疗卵巢储备功能降低(DOR)的临床疗效。方法 80例DOR患者随机分为2组,每组40例。治疗组予益肾化瘀方,每日1剂,早晚分服。对照组予脱氢表雄酮,每次1粒,每日3次。2组均连续观察3个月经周期。检测血清基础卵泡刺激素(b FSH)、基础雌二醇(b E2)、黄体生成素(LH)及血清抗苗勒氏管激素(AMH)水平;阴道超声监测基础卵泡数,测量卵巢动脉收缩期峰值流速(PSV);进行生活质量评价并测量基础体温。结果治疗组总有效率为85.00%(34/40),对照组为62.50%(25/40),2组比较差异有统计学意义(P<0.05)。治疗后2组b E2、b FSH水平均降低,AMH水平、卵巢窦卵泡数、PSV升高,与治疗前比较差异有统计学意义(P<0.05)。治疗后,治疗组b E2、b FSH水平均低于对照组,AMH水平、卵巢窦卵泡数、PSV值均高于对照组,差异有统计学意义(P<0.05)。治疗组基础体温复常率为77.50%(31/40),对照组为52.50%(21/40),2组比较差异有统计学意义(P<0.01)。治疗后2组生存质量、日常活动、健康状况及自我感觉4个维度评分及总分均明显提高(P<0.01),治疗组高于对照组(P<0.01)。结论益肾化瘀方可改善DOR患者卵巢储备功能,提高患者生活质量。

Objective To observe the clinical efficacy of Yishen Huayu Decoction for decreased ovarian reserve(DOR).Methods Eighty cases of DOR were randomly divided into2groups,with40cases in each group.Thetreatment group was treated with oral administration of Yishen Huayu Decotion,one dose a day.The control groupwas given DHEA,a time of1tablet,3times a day.2groups were observed for3menstrual cycles.The levels of basicfollicle stimulating hormone(bFSH),basal estradiol(bE2),luteinizing hormone(LH)and serum anti-Müllerianhormone(AMH)were measured.The number of basal follicles and the peak systolic velocity(PSV)were monitoredby transvaginal sonography.Quality of life assessment and basal body temperature were measured.Results The totaleffective rate was85.00%(34/40)in the treatment group and62.50%(25/40)in the control group,with statisticalsignificance(P<0.05).After treatment,the levels of bE2and bFSH decreased,and the levels of AMH,ovarian antralfollicles and PSV increased,which were significantly different from those before treatment(P<0.05).After treatment,the levels of bE2and bFSH in the treatment group were lower than those in the control group,and the levels of AMH,ovarian antral follicle and PSV were higher than those in the control group,with statistical significance(P<0.05).Therecovery rate of basal body temperature was77.50%(31/40)in the treatment group and52.50%(21/40)in the controlgroup,with statistical significance(P<0.01).After treatment,the scores of four dimensions of life quality,dailyactivities,health status and self-perception were significantly improved(P<0.01),and the treatment group was higherthan the control group(P<0.01).Conclution Yishen Huayu Decotion can improve the ovarian reserve function and thelife quality of patients with DOR.
