老年性急性胆囊炎伴胆总管结石开腹手术时机分析

Analysis of operation timing for open surgery in elderly patients with acute cholecystitis complicated with choledocholithiasis

目的:探析不同手术时机对老年性急性胆囊炎伴胆总管结石开腹手术效果的影响。方法:收治老年性急性胆囊炎伴胆总管结石患者108例,根据手术时机分为3组,早期组给予早期手术,中期组给予中期手术,晚期组给予晚期手术,比较3组并发症发生率及术后化脓性胆囊炎、坏疽性胆囊炎发生率。结果:3组并发症发生率比较,差异无统计学意义(P>0.05)。3组术后并发化脓性胆囊炎发生率比较,差异有统计学意义(P<0.05)。3组术后坏疽性胆囊炎发生率比较,差异有统计学意义(P<0.05)。结论:早期开腹手术可缩短老年性急性胆囊炎伴胆总管结石的住院时间及手术时间,降低并发症发生率。

Objective:To investigate the effect of different operation timing on the open surgery in elderly patients with acutecholecystitis complicated with choledocholithiasis.Methods:108cases of elderly patients with acute cholecystitis complicated withcholedocholithiasis were divided into three groups according to the operation timing.Early group was given the early operation.Middle group was given the middle operation.Late group was given the late surgery.The incidence of complications,incidence ofpostoperative suppurative cholecystitis,gangrenous cholecystitis in the three groups were compared.Results:There was nostatistically significant difference of the incidence of complications among the three groups(P>0.05).There was statisticallysignificant difference of the incidence of postoperative suppurative cholecystitis among the three groups(P<0.05).There wasstatistically significant difference of the incidence of postoperative gangrenous cholecystitis among the three groups(P<0.05).Conclusion:Early operation could shorten the hospitalization time and operation time of elderly patients with acute cholecystitiscomplicated with choledocholithiasis and reduce the incidence of complications.
