探讨肾混合性上皮间质肿瘤的多层螺旋CT表现及病理分析

MSCT scan diagnosis of the mixed epithelial and stromal tumor of the kidney and pathology

目的探讨肾混合性上皮间质肿瘤(mixed epithelial and stromal tumor of the kidney,MESTK)的影像学及病理学特点。方法报告3例MESTK的女性患者,多层螺旋CT平扫及增强扫描可见不规则囊性病变,边界清楚,局部凸向肾盂,病灶多囊状,囊壁实性密度,囊内密度较均匀,增强扫描囊内无强化,囊壁呈中等程度强化,延迟扫描持续强化,术后病理为右肾混合性上皮间质肿瘤。结果肾混合性上皮间质肿瘤具有较特异的影像学表现,结合本病例及以往文献报道病例,MESTK的CT扫描肿瘤呈多房囊实性,部分可凸向肾盂内。所有病例术后随访1年,未见复发及转移。结论肾脏混合性上皮间质瘤临床上罕见,以女性多见,无特征性临床表现,生物学行为大部分为良性,多层螺旋CT平扫及增强扫描具有一定诊断价值。

Objective To discuss the clinic-pathological features and CT findings characteristics of mixed epithelial and stromal tumor of Thekidney(MESTK).Methods3case of MESTK which occurred in female.MSCT scan showed irregular shaped lumps,clear boundary,multiple cystic,wall was solid density,intra-capsular density was uniform,After enhanced scan,intra-capsular density did not change,wall showed moderate enhancement,in delay enhanced scan,its density did not significant decline.Results Imaging expression has a particular characteristic to mixed epithelialand stromal tumor of the kidney.Reviewed the reported cases and this one,MESTK consisted of multiple cystic and solid areas on CT scan,point to renal pelvis.There was no evidence of recurrence and metastasis during the follow-up of one year.Conclusion MESTK is a rare tumorwhich predominantly affects woman with benign biological behavior and no specific clinical feature in most cases.MSCT scan and enhanced scanhas a high diagnostic value to MESTK.
